POSITION SUMMARY
- The Adult Family Home (AFH) Caregiver helps create a joyful & vibrant community by caring for residents in an environment that promotes resident rights, dignity, and freedom of choice, and by providing individualized attention and interaction that addresses all six areas of well-being.
- The AFHC answers resident call lights promptly and administers and/or assists with the self-administration of routine and non-routine and non-injectable medications by oral, rectal, or topical means in accordance with physician orders and nursing practices as directed by the Director of Nursing Services.
ESSENTIAL JOB FUNCTIONS
- Understands and ensures that individualized care plans are followed and provides the AFH care manager or other care planning staff with specific information, changes and observations of the residents' needs and preferences.
- Engages residents with life-enriching activities and helps prepare delicious homemade meals for residents.
- Follows the ADLs of each individuals care plan attending to the individual needs of residents including assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, or other needs in keeping with an individual's care requirements.
- Provides care that maintains each resident's skin integrity to prevent pressure ulcers, skin tears, and other damage by changing incontinent residents, turning, repositioning immobile residents, and applying moisturizers to fragile skin.
- Protects the personal belongings of each resident including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing, memorabilia, etc.
- Promptly reports missing items according to established policy.
- Performs various tasks assigned by the AFH Care Manager including vital signs, residents' weights, applying creams/ointments, collecting specimens, as well as lifting, moving, and transporting residents, using proper body mechanics or lifting devices for accident prevention.
- Responsible for keeping records documenting care provided or other information in keeping with department policies while performing all job responsibilities in accordance with the policy and procedures including standard precautions, thorough hand washing, use of gloves where indicated, and proper disposal of soiled materials.
- Communicates and interacts effectively and tactfully with the resident, visitors, families, peers, and supervisors.
- Greets residents, visitors, and staff courteously and in a manner that respects dignity, confidentiality standards and promotes individual rights.
- Caregivers are responsible for following standard precautions and safe work practices, established fire/safety/disaster plans, risk management, and security; reporting and/or correcting unsafe working conditions, equipment repair and maintenance needs.
- Completes simple nursing treatments and monitors residents' vital signs as directed by the Director of Nursing where State Nursing Regulations authorize such delegation.
- Maintains accurate record of medications and treatments administered or omitted on each resident's clinical record to include the reason medication was omitted or prn medication was given.
- Records narcotics administered or received on the appropriate controlled drug count sheet reporting all medication errors immediately.
- Reports and records changes in resident conditions, incidents and accidents to the AFH Care Manager, physician and family members.
- Routinely communicates with physician, pharmacy, and family as indicated to insure medications are received in a timely manner.
- Provides direct resident care while promoting resident dignity, independence, and choice, and by regularly observing resident's physical and mental condition and directly reporting to the AFH Care Manager.
- Assists with or performs ADL's, takes vital signs and weights, accurately documents care services provided in observation on communication logs and incident reports.
- Properly restores medication and equipment and keeps the medication room clean and orderly.
- Removes outdated or discontinued medication for return to the pharmacy.
- Attends and participated in training, in services, and staff meetings.
- Performs other tasks/duties as assigned.
EXPERIENCE, QUALIFICATIONS & SKILLS
- A high school diploma or equivalent.
- Current WA State CNA/HCA license in good standing.
- Mental Health Specialty Training preferred.
- Must hold a medication pass certification per WA state requirements.
- A minimum of three months' prior Nursing Assistant experience in long-term care preferred.
- Current First Aid and CPR card.
- Food Handlers Card Pass.
- A BCCU Criminal Background Check and fingerprint check.
- Follows and supports community fire safety and infection control practices and promptly reports any unsafe conditions or equipment to the Director of Nursing Services.
